EXPRESS CONSENT DECLARATION FORM OF DATA OWNER

In accordance with the Personal Data Protection Law No. 6698; I hereby accept, declare and undertake in advance that my personal data stated below is collected, recorded, processed, saved, shared with other persons and organizations under relevant legislations by Koleksiyon Mobilya Sanayi Anonim Şirketi ("Company”) within the framework of the legal principles within the scope of the considerations in the clarification text belonging to this text. 

I hereby accept, declare and undertake that my personal data that can be processed by the Company only with my express consent is limited to the data relating to identity card data (name, surname, date of birth, gender, TR identity number) ,e-mail address, residential address, location, phone number, voice record, camera record, website use data, shopping history, survey information, campaign participation information, IBAN information, bank or credit card data (number, expiry date and cvv), shop visits, internet site records, user session information and they are saved for the period stated in the clarification text, and all my data listed above can be transferred to foreign payment organizations, banks, cargos, product suppliers, product sale channels, business partners for analysis and marketing-advertisement purposes.

 My data stated above can be processed provided that it is limited to the considerations stated in the clarification text which is an annex of this express consent text.
